As a Care Team Coordinator, you will:
as the initial point of contact for all interactions with care team members and administrative employees. Ensure adherence to Company policies associated with record establishment, retention, maintenance, and confidentiality.
and enter data for new/current patients and employee records and/or documentation. Contact team members and administrative employees to obtain and maintain current information according to Company policy and state and federal regulations.
clinical records for new patients upon receipt of referral. Complete quantitative administrative review of clinical records at start of care and upon patient discharge. File incoming documents daily. Maintain a control procedure for active patient files. Maintain a tracking system to ensure clinical records are current according to Company policy and regulatory requirements.
employment files for new employees upon receipt of information. Maintain a tracking system to ensure employment files are current according to Company policy and state and federal regulations. Perform data entry of new employee information and inactivate employment files as necessary per Company policy.
medical supplies for care team in accordance with physician orders and Company policy.
availability lists and complete/accurate schedules, including frequency and duration of services as identified in the plan of care and under the direction of the Clinical Manager or designee. Update schedules as changes occur and follow up with any identified deficiencies in the scheduling process.
Qualifications
School Diploma or equivalent
inimum of one year data entry, word processing and/or medical records maintenance experience in a medical customer service environment generally required
旳ttention to detail, along with excellent organizational, oral and written communication, customer service, and problem-solving skills
