Job Summary:
Division/Program Description: FBHS is responsible for planning, development, evaluation, and oversight of forensic discharge planning and community based services including the Medication Grant Program. The goal is to ensure that all consumers who leave the criminal justice system have access to necessary community based mental health services.

The Discharge Planning Unit at Rikers Island is designated to assist individuals who are receiving mental health treatment while incarcerated in the New York City Correctional System with obtaining necessary services to continue their treatment after release. These services include, but are not limited to issuing referrals and scheduling appointment referrals/appointments to community mental health providers and assistance with obtaining Medicaid and Public Assistance.

- Responsible for scheduling, training, and disciplining all assigned staff.
- Identify and assist in resolving system problems that affects inmates at discharge time.
- Ensure that all functions are covered in cases of planned or unplanned staff absences.
- Participate with a treatment team at discharge planning meetings of inmates.
- Attend regular meetings with Mental Health to discuss clinical issues and development of plans based on clinical needs.
- Coordinate jail based discharge-planning functions with Mental Health Unit Chief. Identifying new Brad H class members to be case assigned inclusive of chart review.
- Participate in treatment team meetings and facilitating the timely transfer of documents.
- Supervise, monitor, and assign, the Discharge Planning caseloads.
- Provide clinical and administrative supervision of all planning staff.
- Review, approve and ensure that staff expeditiously identify and secure appropriate community placement for the Brad H class members.
- Review tickler system which direct what services need to be completed.

PREFERRED SKILLS:
Experience in Mental Health, case management, and social work. Strong organizational and management skills. Must have good communication and interpersonal skills. Knowledge of entitlement, community mental health resources and substance abuse. The ability to provide direct services to mentally ill individuals and linking to a broad range.

Experience Required:
A Baccalaureate degree from an accredited college and five (5) years of full-time professional satisfactory experience acquired within the last fifteen years, in a health services setting such as a laboratory, hospital, or other patient care facility, or in a public health, environmental health, or mental hygiene program, at least 18 months of which must have been in a managerial capacity, consisting of managerial experience clearly demonstrating the ability to perform difficult and responsible managerial work, requiring independent decision-making concerning program management, planning, allocation of resources, and the scheduling and assignment of work; or
2. Education and/or experience equivalent to (1) above. Education may be substituted for experience on the basis that each 30 graduate semester credits from an accredited college in hospital administration, public health, public administration, business administration, management or administration can be substituted for one year of non-managerial experience up to a maximum of 60 semester credits for two years. However, all candidates must have a minimum of a Baccalaureate degree and 18 months of managerial experience as described in (1) above.
